After setting up your first appointment, You will get an email titled Thanks for Booking. There is a button to fill out the intake form in that email. Please fill out your online intake form completely so that we have thorough information about your health history. If you don’t receive a link to the intake forms, please do let us know and we will get them sent out right away!
